Down to your county: the federal rules and North Carolina numbers above are the same across the state. County-level detail — your county's plan list and local help — is coming soon; those figures aren't in our data yet, so we don't show them. How Medicare works in North Carolina

The four parts are federal — identical in North Carolina and every other state. What changes here is which plans your county sells, the Medicaid limits that lower your costs, and how the free counseling program runs.

Part A
Inpatient hospital, skilled nursing, hospice, some home health. Most people pay no premium.
Part B
Doctor visits, outpatient care, prevention. The standard 2026 premium is $202.90/mo; higher incomes pay an IRMAA surcharge. IRMAA calculator →
Part C
Medicare Advantage — a private bundle of A + B, usually with drug coverage, through a network. We never name or rank specific plans.
Part D
Prescription drugs — as a stand-alone plan or built into an Advantage plan.

How North Carolina compares

North Carolina — Advantage share58%
U.S. national average51%

More of your neighbors choose Advantage than the national norm. That doesn't make it the right pick for you — Original Medicare + Medigap trades a higher premium for go-anywhere freedom. It's a fit-to-your-situation choice. CMS Monthly Enrollment · 2026 (source, opens in new tab)

Medigap in North Carolina — the six-month window that matters

North Carolina uses the federal standardized system: Plan G is Plan G, insurer to insurer — only price and service differ. Your protection is timing: for six months after you are both 65 and on Part B, no insurer can turn you down or charge more for your health history. We never name or price policies; SHIIP can show you current North Carolina rates for free.

North Carolina HIV SPAP — extra prescription help

North Carolina's own drug-cost program works alongside Medicare Part D. Disease-specific only — HIV. No general senior drug program. Eligibility and benefits change — confirm current details with the program or your SHIIP counselor.

PACE — nursing-home-level care, at home

The Program of All-Inclusive Care for the Elderly keeps people who need that level of care in their own communities. North Carolina has about 11 PACE programs. You must be 55+, live in a service area, and be certified as needing that level of care. Details at Medicare.gov (opens in new tab).

North Carolina Medicare: common questions

When can I enroll in Medicare in North Carolina?

Timing is federal, so it is the same as everywhere: a 7-month Initial Enrollment Period around your 65th birthday, then the Annual Enrollment Period each October 15 – December 7. Missing Part B without qualifying coverage creates a permanent penalty.

Does North Carolina have its own Medicare rules?

No. Eligibility, enrollment windows, and penalties are identical nationwide. What varies is which plans your county sells, the Medicaid income limits for Medicare Savings Programs, Medigap consumer protections, and how SHIIP operates.

Can North Carolina help me pay my Medicare premiums?

Yes, if your income is limited. Medicare Savings Programs — QMB, SLMB, QI — can pay your Part B premium and, with QMB, your deductibles and cost-sharing. Qualifying also gives you Extra Help with drug costs automatically. Apply through North Carolina Medicaid or ask a SHIIP counselor.

How many Medicare plans are available in North Carolina?

North Carolina has roughly 203 Medicare Advantage plans statewide, though the number in your specific county may be higher or lower. We do not name, compare, or recommend specific plans — see every plan in your ZIP on the official Medicare Plan Finder at Medicare.gov.
