How Medicare works in Illinois

The four parts are federal — identical in Illinois and every other state. What changes here is which plans your county sells, the Medicaid limits that lower your costs, and how the free counseling program runs.

Part A
Inpatient hospital, skilled nursing, hospice, some home health. Most people pay no premium.
Part B
Doctor visits, outpatient care, prevention. The standard 2026 premium is $202.90/mo; higher incomes pay an IRMAA surcharge. IRMAA calculator →
Part C
Medicare Advantage — a private bundle of A + B, usually with drug coverage, through a network. We never name or rank specific plans.
Part D

Medigap in Illinois — the six-month window that matters

Illinois uses the federal standardized system: Plan G is Plan G, insurer to insurer — only price and service differ. Your protection is timing: for six months after you are both 65 and on Part B, no insurer can turn you down or charge more for your health history. Illinois Cares Rx — extra prescription help

PACE — nursing-home-level care, at home

When can I enroll in Medicare in Illinois?

Timing is federal, so it is the same as everywhere: a 7-month Initial Enrollment Period around your 65th birthday, then the Annual Enrollment Period each October 15 – December 7. Missing Part B without qualifying coverage creates a permanent penalty.

Does Illinois have its own Medicare rules?

No. Eligibility, enrollment windows, and penalties are identical nationwide. What varies is which plans your county sells, the Medicaid income limits for Medicare Savings Programs, Medigap consumer protections, and how SHIP operates.

Can Illinois help me pay my Medicare premiums?
